Six weeks is enough time to build real command of the SPI content outline if your study time is structured around how the exam is actually weighted — not spread evenly across topics regardless of how often they appear. This plan follows the official domain weightings: 23% Perform Ultrasound Examinations, 7% Manage Ultrasound Transducers, 26% Optimize Sonographic Images, 34% Apply Doppler Concepts, and 10% Provide Clinical Safety & Quality Assurance.
Adjust the pacing to your own schedule — this assumes roughly 10-15 hours of study per week, which is a reasonable target for someone studying alongside clinical rotations or a job, but not the only way to structure it.
Week 1: Foundations
Start with the fundamentals that everything else builds on — frequency, wavelength, propagation speed, attenuation, and the piezoelectric effect. Don't try to connect these to Doppler or resolution yet; just build a solid base. End the week by reviewing the full ARDMS content outline so you have a mental map of what's coming.
Week 2: Transducers and Perform Ultrasound Examinations
Cover Domain 2 (Manage Ultrasound Transducers, 7%) in the first half of the week — it's compact and pairs naturally with the piezoelectric concepts from week 1. Then move into Domain 1 (Perform Ultrasound Examinations, 23%): patient care, sound-tissue interactions, gray-scale artifacts, bioeffects, and 3D/4D and contrast concepts.
Weeks 3-4: Optimize Sonographic Images
This domain (26%) gets two full weeks because of both its weight and its density — axial, lateral, elevational, and temporal resolution; frame rate and scan line tradeoffs; TGC and gain; harmonic imaging; dynamic range; spatial compounding; and M-mode. Spend week 3 on resolution concepts and basic image optimization tools, and week 4 on the more advanced optimization concepts (harmonics, compounding, edge enhancement) plus a cumulative review connecting resolution to PRF and frame rate.
Week 5: Apply Doppler Concepts
Doppler is 34% of the exam — the single largest domain — and it depends heavily on the PRF and frequency concepts from earlier weeks, so this is the right point in your schedule to tackle it; the foundation is in place. Cover the Doppler equation and angle dependence, aliasing and the Nyquist limit, wall filters, sample gate concepts, CW versus PW Doppler, color and power Doppler, spectral waveform analysis, and gain/scale settings.
Given the weight of this domain, if you find yourself short on time in week 5, it's reasonable to borrow study hours from earlier weeks' review time rather than compress Doppler — but don't skip the connections back to PRF and frequency from weeks 1-4; those connections are where many exam questions live.
Week 6: Clinical Safety & QA, plus full review
Start the week with Domain 5 (Provide Clinical Safety & Quality Assurance, 10%) — infection control, QA documentation, transducer and machine integrity checks, phantom testing, and statistical concepts like sensitivity and specificity. This domain tends to be more memorization-based and is a good fit for a focused couple of days.
Spend the remainder of the week on full-length, timed practice exams under realistic conditions — ideally at least two or three across the week, with review sessions after each to identify any remaining weak areas. In the final 1-2 days before your exam, shift focus toward the highest-weighted domains (Doppler and image optimization) for a final review pass, since these represent the largest share of points and are worth the extra attention right up to test day.

If you're working with less than six weeks: don't try to compress this schedule proportionally across all domains — instead, protect the time allocated to weeks 3-5 (Optimize Sonographic Images and Apply Doppler Concepts, 60% of the exam combined) first, and compress weeks 1, 2, and 6 if you need to find time. A shorter plan that still gives Doppler and image optimization their due is better than a "complete" plan that shortchanges 60% of the test.
